Before you go to your appointment, print out the below forms and complete pages 1 & 2 of the Medical Examination Report From. The doctor or nurse practitioner will complete the Medical Examination Certificate at the end of your appointment.

Once you have a complete Medical Examination Report Form AND a complete Medical Examination Certificate, please forward a copy of both to Jennifer Hunter in the Board Office.
As always, the best and secure way to make sure NJMVC gets your Medical Examination Report Form is to actually take it to an NJMVC office (the office in North Cape May is taking these forms and processing them).
