For the safety of our staff, students, and their loved ones, all Dennis Township Schools' families must complete this COVID-19 related questionnaire. The form must be completed on the morning of EVERY in-person learning day for your child, until further notice. EACH MORNING of in-person learning, please complete a separate form for each child BEFORE they board the bus or BEFORE drop off at school. The form is located on our district website www.dtschools.org under the PARENTS tab in the main menu, then select COVID SCREENING FORM. If you have any questions, please call the school.
